Case Management in Vermont

We also know that achieving your goals depends on the people who support you, so we make sure our team comprises the most knowledgeable, skilled, dedicated, and passionate Case Managers available.

We provide statewide Case Management, (Basic, Standard, and Intensive) and Brain Injury Program (BIP) services to Vermont through the Medicaid-funded Home and Community Base Services waiver (HCBS) which is managed by the Developmental Disabilities Services Division (DDSD). HCBS services are tailored to the individual’s specific needs and based on individualized budget and Individualized Support Agreement (ISA).
