We are here to help you maintain your independence
We understand that you want to remain at home as long as possible; however, sometimes the tasks of daily living become more challenging. We are here to help you put needed supports in place so that you can remain as independent and feel as supported as possible in your home.
What is the waiver?
Formally known as the CCSP (Community Care Services Program), the waiver helps individuals who are elderly and/or have functional physical challenges, enabling them to remain in their homes instead of opting for nursing home care. This waiver provides support to elderly and disabled Georgians.
EDWP provides assistance in the following areas:
- Adult day health
- Alternative living services
- Emergency response services
- Home-delivered services
- Skilled nursing services, as well as physical, speech and occupational therapy
- Personal support services or extended personal support services
- Consumer-directed personal support services (consumer hires and supervises workers)
- Out-of-home respite care
- Home delivered meals
To qualify, you must be eligible for Medicaid and:
- Be at least 65 years old (or, if younger than 65, meet certain disability requirements)
- Be impaired by physical limitations
- Choose to receive services in your home or community instead of a nursing home
- Participate in only one waiver program at a time
We serve in the following Georgia counties: Baker, Colquitt, Calhoun, Dougherty, Decatur, Early, Grady, Lee, Miller, Mitchell, Terrell, Thomas, Seminole and Worth.
