How does C-Change™ work?
Columbus’ healthcare team bridges boundaries and bring together the skills of professionals to assess, plan and manage care jointly. Based in the community, staff work proactively to support individuals’ goals and ensure that quality, comprehensive, continuous care services are delivered.
Columbus trains staff to utilize screening and assessments tools as required by stakeholders and evidenced-based tools for identification of risk factors. Assessments, screenings, and prescriptive algorithms can be integrated and applied to member/participant interactions to identify needs and risk triggers and aid in the stratification of acuities.
Columbus’ History in Complex Care
Columbus is the country’s leading provider of traditional, complex medical, behavioral, Acquired Brain Injury (ABI), ABI Long Term Care (LTC), Head and Spinal Cord Injury (HASCI), and Aged and Disabled (A&D) care coordination services. We have experience with providing different levels of care management services, from episodic case management and support coordination to intensive care management services that emphasize the importance of individually tailored supports and valued range of person-centered approaches to holistic care.
Columbus places a high value in the competency training of our staff, so our staff are provided extensive, on-going education to have the knowledge and understanding to successfully work with individuals of all abilities. Our curriculum is grounded in evidence-based practices that directly guide the development and maintenance of strategies.

Elderly and Disabled Waiver Program (EDWP)
Formerly known as the Community Care Services Program (CCSP), the waiver helps individuals who are elderly and/or functionally have physical challenges to remain in their homes as an alternative to nursing home care. The Columbus Organization currently offers support coordination services through the EDWP program in the state of Georgia.
